Introduction: Cerebral palsy usually presents with incomplete movements with abnormal reflexes, loosening and stiffening of the arms and legs and upper body, abnormal structure, involuntary movements, unbalanced gait or a combination of the problems mentioned above. Treatment and rehabilitation plans and various methods to treat and help restore life are being explored. Method: In this study, four children with cerebral palsy with movement disorders and one healthy subject were studied. Motion data were received and stored by Vicon motion analysis system and then the necessary modeling were performed in Opensim software to evaluate the interventions. The function of botulinum toxin was simulated by muscle weakness which was ۵۰% attenuation in these subjects. Finding: The diagrams obtained from patient model simulations and surgical and injection simulations were compared and reported. The rectus femoris transfer group is significantly different from the botulinum toxin group (P <۰.۰۰۱) and the patient's initial model and is close to the healthy model. Conclusion: The results show that starting surgery in the first stage is more effective than botulinum toxin and muscle weakness with botulinum toxin injection may not be effective in the short term and requires planned studies over long periods of time or further weakening of the target muscle. In future research, the results can be re-examined by modeling further muscle weakness.
