Helicobacter pylori, Hp, is the most important causative agent of chronic active gastritis and peptic ulcer disease. Its role as a risk factor for gastric carcinomas has particular importance. The aim of this study is to determine the correlation of the strain variation and degree of homogeneity of infecting Hp with their disease outcomes. Thus Hp strains are isolated from different gastric sites of 14 cancer (eight parts each) and 53 (four parts each) non-ulcer dyspeptic subjects. Following the identity confirmation of Hp by urease test, culture and PCR amplification of ureC gene, the genetic heterogeneity of the isolated strains from different parts of the stomach )gastric mapping) are evaluated by RAPD-PCR method. RAPD-PCR profile of each site is compared and single or multiple colonization is determined for each patient. Using this technique, more than one colonizing strain was found in six out of 14 (42.9%) gastric cancer patients.Among the non ulcer dyspeptic patients 14 out of 53 (26.4%) individuals demonstrated multiple strain colonization. In Conclusion, this comparative study reveals that in Iran, where Hp colonization is relatively high, the rate of multiple strain infection is also higher than that of developed countries (Europeans and North Americans) and even more than some developing countries.
