Introduction: Early diagnosis of infections lead to proper treatment and helps to prevent unnecessary use of broad-spectrum antibiotics and drug resistance. The aim of this study was comparison of predictive value of the absolute neutrophil count, absolute band count and toxic granulation of neutrophils with quantitative CRP ininfectious disease leading to admission in farshchian hospital of Hamadan. Material and Methods: In this study, 200 cases were selected from admitted patients in Farshchian hospital in years 2015-2016 due to urinary tract infection, pneumonia, soft tissue infection, septic arthritis, sepsis and dysentery. Before treatment, CBC(diff) and quantitative CRP was done and the percentage of neutrophils, the percentage of band cells and toxic granulation was evaluated with peripheral blood smear by pathologist.This informations with demographic data were collected by the check lists and were analyzed with SPSS software version 16. Results: The mean age of the patients was 56.26 years. 60.5 percent were men and 39.5 percent were women. The most frequent cause of hospitalization were pneumonia, soft tissue infections, sepsis, urinary tract infection, septic arthritis and dysentery. The correlation coefficient between the absolute neutrophil count, absolute band count, toxic granulationand CRP was 0.43, 0.47 and 0.47 respectively. Conclusion: The results of study showed that there was a linear correlation between the absolute neutrophil count, absolute band count, toxic granulation and CRP and this variables can be replaced instead of quantitative CRP. This parameters are valuable in the diagnosis of patients with the high level of CRP with the reason ofnon-infectious disease and in patients who CRP can not be evaluated secondary to liver and kidney diseases. The results of our study showed that the predictive value oftoxic granulation and absolute band count are higher than absolute neutrophil count.
