Determining the Performance of Radiologists Using the Sherpa Technique in Qazvin University of Medical Sciences Hospital

Introduction: The use of human lines in medical life is one of the most important issues in this field, which shows its number, because it is an impossible compensation to protect against accidents. Therefore, it is important to use methods to identify and reduce them. The aim of this study was to perform the practice of radiologists using the Sherpa technique in hospitals affiliated to the Qazvin University of Medical Sciences.Methods: This cross-sectional descriptive study was performed using Sherpa method in ۲۰۲۰. The population and the sample of the study included the safety experts and radiologists. Data were collected through two interviews with and safety experts and radiologists and direct observation through a researcher-made observation checklist. Using the hierarchical analysis method, the sub-tasks were determined and finally the information obtained in the Sherpa worksheet was completed and analyzed using SPSS software.Results: Among the tasks identified, ۲۸ errors were identified, of which the highest error is for functional errors with ۱۱, then visit error ۹, recovery equal to ۵and communication error ۳. The selected error is the lowest value, i.e. zero. Error risk level for unacceptable risks ۳۸.۹۸%, undesirable risks ۳۷.۲۸%, acceptable risks with need for revision ۶.۷۷% and acceptable risks without need for revision was ۱۶/۹۴ %. Finally, after reviewing the error recovery, it was found that ۲۳ errors could not be recovered and ۵ errors could be recovered.Conclusion: Applying risk prevention method to identify potential risks and monitor errors is felt. Hospital officials should pay special attention to functional and visit errors in hospital occupations. In this regard, by developing a coherent program in order to establish national accreditation standards in the field of error management, take a step to identify and control the factors predisposing to human error in the hospital.
