Updates on The Prognostic Value of BaselinePET/CT Scan Parameters in Diffuse Large B Cell Lymphoma:A Systematic Review

Introduction: Diffuse large B cell lymphoma (DLBCL) is ahematologic malignancy that included about ۲۵-۳۰% of non-Hodgkin’s lymphoma. Relapse or refractory disease occurs inabout ۳۰% of initial-chemotherapy treated DLBCL patients.The positron emission tomography/computed tomography(PET/CT) scan is a practical tool for DLBCL staging and prognosisprediction. Several studies on these patients showed thatdifferent parameters of baseline PET/CT could predict overallsurvival (OS) and progression-free survival (PFS) in the pretreatmentsetting. In this regard, this study aims to evaluate theaccuracy of different baseline PET/CT parameters for DLBCLoutcomes prediction which may lead to prognosis-based treatments.Materials and Methods: A systematic search has been donein MEDLINE/PubMed, Scopus, and Web of Science databasesdatabases,looking for recent experiments using baseline PET/CT forprediction of prognosis in DLBCL. Duplicated and irrelevantpapers were excluded. Finally, ۳۵ articles were included in thisstudy.Results: Our study showed that shorter OS and PFS are associatedwith high Metabolic Tumor Volume (MTV), Total LesionGlycolysis (TLG), Maximum Tumor Dissemination (Dmax),and maximum standardized uptake values (SUVmax) in pretreatmentPET/CT in descending order.Conclusion: Baseline PET/CT can be utilized for the predictionof prognosis and treatment outcomes. More invasive initialtreatments could be considered for patients who are at highrisk for recurrent or refractory disease. Besides, it is proposedthat a combined model with PET/CT, International PrognosticIndex (IPI), and genetic assessments (especially the presenceof BCL۲/MYC rearrangements) using artificial intelligence hasthe most promising accuracy for achieving the mentioned goalof this study.
