Respiratory Manifestations of COVID-۱۹ in Children: A Narrative Review

Background: Coronavirus disease ۲۰۱۹ (COVID‐۱۹, the seventh coronavirus) is a new infectious disease that spread worldwide and rapidly became a pandemic. COVID‐۱۹ is rare in children and often asymptomatic. The respiratory system is mainly involved in COVID-۱۹ infection.
Objectives: This study aimed to review respiratory manifestations in children with COVID-۱۹.
Methods: We searched for studies published from January ۲۰۲۰ to January ۲۰۲۱ in PubMed, Google Scholar, and Scopus in English. The search words were “۲۰۱۹-nCoV,” “novel coronavirus,” “COVID-۱۹,” “SARS-CoV-۲,” “severe acute respiratory syndrome coronavirus ۲,” “children,” “child,” “infant,” “teenager,” “adolescent,” “pediatric,” and “neonate.”
Results: Cough was a common symptom in our study (۱۸.۸%-۱۰۰%, mean=۶۴%). Pneumonia prevalence in children with COVID-۱۹ is ۱۱%-۶۷% (mean=۴۶%). Mild respiratory symptoms (۱۴% - ۶۰%) and severe respiratory distress syndromes (۱۴%) were reported in some studies. Upper respiratory tract infection (URTI) was reported in ۴ studies ranging from ۱۱.۱% to ۳۴.۴% (mean=۲۴.۱%). Ground-glass opacities (GGO) was reported in ۱۲.۲% to ۸۱% of cases (mean=۴۹.۰۱%). Isolated consolidations were reported in ۱۷% and ۵۸% of patients in two studies. Enhanced lung texture was reported in ۱.۳% to ۵۰% (mean=۲۰.۰۷%) of patients. Bilateral patchy shadowing was seen in ۱۲.۳% to ۶۸% (mean=۴۶.۵۸%) of patients.
Conclusions: Cough is the most common manifestation of respiratory system involvement in children with COVID-۱۹. GGO and bilateral patchy involvement are the most common finding on lung CT scans in children with COVID-۱۹.
