The Quadruple Test: A single center experience in 16690 patients

Background: Quadruple Test is a maternal blood screening Test that looks for the abnormal levels of AFP, hCG, Estriol, and Inhibin-A to predict fetal abnormalities in pregnant women. In this study we describe the results of 16690 pregnant women referring to our center. Material and Method: In this descriptive study a number of 16690 pregnant women referring to Boghrat Laboratories, Tehran from 2008 till 2017 included. Among this patients 7426 were in first trimester, 6802 in second trimester, 2464 for third trimester, and 2464 for integrated test. Risk factors including smoking, insulin dependent diabetes, IVF, Down-syndrome, and pre-eclampcia , as well as final screening results were evaluated. Results: Risk factors including smoking, insulin dependent diabetes, IVF, Downsyndrome, and pre-eclampcia were observed in 33, 195, 139, 14, and 4 patientsrespectively. The final result of screening test showed positivity in 851 (5.1%) of the patients and were as follow: increased risk of down syndrome (353 patients), increased risk of pre-eclampcia (203), increased risk of both down s syndrome and pre-eclampcia, raised AFP (112), increased risk of pre-eclamcia and raised AFP (37), previous pregnancy affected with down s syndrome (11), increased risk of Down s syndrome, pre-eclampcia and raised AFP (3), increased risk of trisomy 18 (3), previous pregnancy affected with pre-eclampcia (2), increased risk of trisomy 13 (1), increased risk of trisomy 18 and pre-eclampcia (1), increased risk of Down s syndrome and trisomy 13 (1), increased risk of Down s syndorme and raised AFP(1), and increased risk of down s syndrome, trisomy 13, and pre-eclampcia (1). Conclusion: This study is one of the largest center experiences from Iran and describes abnormal values of Quadruple test elements, frequency of risk factors and screening test results.
