Background :All patients with a blood transfusion request from 2006 to 2017 were studied at the Children s Medical Center Hospital that Includes all departments of the internal, surgical, emergency, emergency care and infants (19 department)Method:antibody screening test was performed for all blood transfusion requests by IBTO panel’s. also in cases where the result of the screening was negative , If there was a request for blood transfusion , An instant cross-match was done But the process continues. Results :In the first study, from 2006 to 2017 , 176696 blood transfusion requests from all 19 department of the hospital, Most requests for surgery and the lowest request for NICU. For all requests antibody screening test was performed.40,940 (23%) of the requests were canceled and 135,756 (77%) blood transfusions were received. The nervous and infectious department have the highest blood consumption and the lowest cancellation rate than the number of requests. 96% and 95% of requests were used respectively.The Emergency department admitted have the lowest blood consumption and the highest cancellation rate than the number of requests. 27% of requests were used .In the second study in 2017, 15310 had a blood transfusion request and a cross-match test was performed for all requests. 11462 (75%) were transfused and 3848 (25%) were not transfused. The C / T Raito average was calculated to be 1.3. Conclusion:In the first study, considering an antibody screening test for all requests, 23% was saved in the consumption of blood. In fact, performing an antibody screening test, in addition to cost-effective and reducing the C / T ratio, increases the speed of blood preparation, especially in emergency situations, and if an operation requires blood transfusion, only one cross-match was performed .In the second study, the C / T ratio with a mean of 1/3 showed the effectiveness of antibody screening and saving blood. This is almost equivalent to a study conducted at the Kenya Children s Hospital in 2016, which was 1.2. Also in another study in the Department of Pediatrics, Birjand Hospital in 2015-2016, the C / T ratio was reported to be 1.5
